Wellhead spills due to packer leaks:
Primary Cause: Excessive stem wear due to scoring, marking or pitting.
Secondary causes: Misaligned rod with respect to pumping shaft, salt or carbonate deposits on string, lack of lubrication due to production cut-off, lack of lubrication due to viscous oil or high water cut-off.
Proposed solutions: Replace the rod, check for leaks, lubricate the rod with emulsion, or use a double packing with a lubricating chamber.
Recommended Tulsa products: Safe-T with Stopleak for leakage control, Mark I for stem lubrication, Autoalign to correct misalignment, and Double Ecologic for double packing.
Wellhead spills due to rod breakage:
Root cause: mechanical damage, material fatigue or corrosion.
Proposed solutions: Replace the clamp, align the sucker rod, or lubricate the rod with anti-corrosion additives.

Primary Cause: Excessive wear of polished rod due to scoring or pitting.

Gas Lock in pump:
Main cause: gas accumulation in the pump due to high back pressure generated by gas in the casing.
Proposed solution: Extract the gas from the casing using an eductor.
